The National Home-based Primary Care and Palliative Care Learning Collaborative holds it kick-off meeting on January 10-11, 2019.  Nine practices were chosen to participate in this National Learning Collaborative from among 17 applicant practices.  The Learning Collaborative will focus on developing a culture of quality improvement in home-based medical care.  Practices will engage in quality improvement activities focused on quality indicators contained in the National Home-based Primary Care and Palliative Care CMS-approved qualified clinical data registry (QCDR).  The 9-month Collaborative will consist of in-person meetings and monthly video-webinars focused on various aspects of quality improvement and culture change.

The participating practices represent a wide range of home-based medical care practices.  The participating practices are: Bassett Healthcare (Cooperstown, NY), Baylor House Call Program (Houston, TX), MD at Home (Chicago, IL), Mercy Advance Care Coordination Program (Grand Rapids, MI), Mount Sinai Visiting Doctors (New York, NY), Northwell Health House Calls (New Hyde Park, NY), Penn Truman G Schnabel in Home Primary Care Program (Philadelphia, PA), Wake Forest House Call Program (Winston Salem, NC), Medstar House Call Program (Washington, DC)
